Wellness Plans
Beginning January 1st, 2022 we are excited to introduce the Touhey Family Dental Wellness Plan. This is a savings plan designed to provide our patients with a convenient, flexible in-house benefit option at an affordable price.
Our Dental Wellness Plan is best suited for our patients who don’t currently have a dental insurance plan or those who wish to end their current dental insurance plan.
PLAN # 1: $475 annual membership fee per individual($720 value)
2 Routine Dental Cleanings per year (non-periodontal)
2 Doctor Exams per year
1 Set of Bitewing X-Rays per year for cavity detection
Any additional routine cleanings for $ 75 per visit
Plan # 2: $695 annual membership fee per individual($920 value)
3 Periodontal Maintenance Cleanings per year
2 Doctor Exams per year
2 Sets of Bitewing X-Rays per year for cavity detection
(20% off any additional periodontal maintenance cleanings per year)
Both membership plans also include the following:
Panoramic X-Ray once every 3 years for $80($172 value)
20% off all restorative procedures done in our office (e.g. fillings, crowns, dentures/partials)
No Deductible
No Waiting Period
No Maximum Benefit Limit
No Pre-existing condition limitations
No Pre-authorization required
Dental Program Guidelines:
Annual Membership dues may be paid in either 1 or 2 installments.
The patient’s portion of the bill is due the day of the service.
Cannot be used in conjunction with other dental plans.
No refunds of premiums will be issued at any time if the participant decided
not to utilize the dental plan.
Cleanings cannot be carried over to the following year.
Outside financing(ex. CareCredit) may not be used to pay the annual fee.
EXCLUSIONS: 20% Discount does NOT apply to Invisalign Treatment and TMJ Splints/Sleep Appliances that are already discounted by Medical Insurance.
